Position: Certified/Registered Nurse Practitioner

Reports to: Supervising Physician and Practice Administrator

Position Purpose: This position is responsible for providing cardiovascular care services, including assessing and managing patients following established standards and practices.  Compassionate, high quality cardiovascular care will be met by assuring that the needs of the patients, physicians, and other health care providers are met in a timely and courteous manner.

Qualifications:

            Education:

1)     Master’s Degree in Nursing

Certification/License:

1)     Licensure as a registered nurse in the State of Wyoming

2)     Licensure as an Advanced Registered Nurse Practitioner in the State of Wyoming

3)     Certification as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP)

4)     Current ACLS certification

5)     Licensure with Drug Enforcement Agency

Experience:

1)    Three years of professional nursing experience in a cardiology setting, desired.

2)    Two years of experience as ARNP preferred.

Duties and Responsibilities: 

1. Obtains comprehensive medical history and performs complete physical examinations for patient needs. 
2. Performs problem-focused exams on patients presenting with acute and chronic illness.
3. Orders appropriate diagnostic tests, treatments, medications and referrals.
4. Interprets and follows-up on test results, response to treatments, and medications and consultant’s recommendations.
5. Establishes a diagnosis, formulates a plan of care, evaluates the response to the plan, and revises the plan as needed, collaborating with or referring to a physician as needed. 
6. Collaborates with the physician in managing acute and long-term medical needs of the patient.
7. Initiates appropriate evaluation and management for emergency situations.
8. Assists the physician in obtaining informed consents.
9. Initiates admissions orders per protocol and/or in collaboration with the physician.
10. Coordinates discharge follow-up.
11. Interviews and counsels patients regarding health and illness prevention, recommends community resources to meet patient or family needs, instructs patients and family regarding medication and treatment plan, and provides patient education.
12. Documents care provided and patient response in an accurate and timely manner in accordance with legal requirements, professional standards and clinic policies.  Assures documentation supports codes assigned.  Actively pursues inconsistencies and/or inaccuracies in coding and/or billing.  Maintains and review patient records. 
13. Maintains awareness of own safety in carrying out duties and responsibilities.  Adheres to universal precautions and infection control policies and procedures. 
14. Provides monitoring and continuity of care between physician visits. 
15. Follows Wyoming Cardiopulmonary Services, PC policies and procedures. 
16. Maintains patient confidentiality, follows HIPAA guidelines, and uses discretion in patient interactions. 
17. Promotes a positive, professional image of Wyoming Cardiopulmonary Services, P.C., both internally and externally.  Promotes positive interpersonal relations.
18. Assists with nuclear treadmills and stress tests and needed.  
19. Computer access is limited to job description duties and responsibilities (password changes every 90 days).  A confidentiality agreement is signed by all employees of the practice. 
    
    

Other Duties and Responsibilities:

1. Assists in educational development of back office staff. 
2. Serves as role model and resource for nurses and medical assistants.
3. Participates as needed in research protocols and practice.
4. Attends meetings and participates in committees as requested. 
5. Participates in professional development activities and maintains professional affiliations. 
6. Accepts special assignments as directed by Wyoming Cardiopulmonary Services, PC. 
   
   

Supervision:

1.     The collaborative physician or his/her designee shall be available at all times either on site or by telephone if consultation is needed for any reason.

2.     Physician involvement shall be sought for, but not limited to the following situations:

                        i.         Initial evaluation of a cardiac patient

                       ii.         Any uncommon, unfamiliar, or unstable problem.

                     iii.         Any unexplained physical examination or historical findings.

                     iv.         Any patient conditions that do not fit the commonly accepted diagnosis patterns for a disease or disorder.

                       v.         Patients that request MD follow-up only.

                     vi.         An emergency situation after stabilizing care has been initiated. 

Evaluation:

1.     The collaborative physician and the ARNP shall review patient issues and concerns on a PRN basis.

2.     The ARNP and the collaborating physician shall informally evaluate the ARNP care and documentation of patient encounters on an ongoing basis.

3.     The collaborative physician of Wyoming Cardiopulmonary Services, PC shall formally evaluate the ARNP’s practice after the first six months and then annually. 

Environmental and Physical Demands:

1.     Requires ability to drive if needed to various clinics.

2.     Requires normal visual acuity and hearing.

3.     Requires exposure to communicable disease and bodily fluids.

4.     Requires standing and walking for prolonged periods of time.

5.     Requires full range of body motion, including handling and lifting patients, manual and finger dexterity and eye-hand coordination.
