Job Summary: It is the responsibility of the RN Field Supervisor to supervise, instruct, and guide the HHAs in the delivery of quality home health care services, conduct assessments and home visits of patients on home care program.
Supervises, instructs, and guides the aides in the delivery of quality home health care services.
Participates in the orientation of field staff.
Provides field supervision for paraprofessional annually and as necessary and provides remediation on-site as required.
Performs skills for home health as needed.
Performs patient assessments, newly hired aide visits, and visits for change in equipment or patient status.
Reviews employee documentation to monitor adherence to Agency’s policies and procedures including accurate documentation.
Reviews aide duty sheets and provides instruction and remediation as necessary.
Submits all documentation according to agency policy.
Performs other duties as assigned.

Direct supervision of field staff assigned to patients.
Implementation and enforcement of agency policy.
Supervisory visits to field staff to promote safe and appropriate patient care and effective staff utilization.
Maintaining patient confidentiality.
Adherence to infection control protocol.
Reporting incidents and occurrences.
Knowledge and compliance of Network policies and procedures.
Adherence to dress code and to arrive to work in clean, neat and appropriate attire.
Knowledge and compliance of New York State DOH Codes and Joint Commission standards.
Maintaining current nursing licensure.
Maintaining current CPR certification.
Demonstrating reliable punctuality and attendance.
Registered Nurse (RN) licensed to practice in New York State.
Baccalaureate Degree preferred, Associates Degree required.
Minimum of one year experience in home care as a community health nurse in a Certified Home Health Agency, Long Term Home Health Care Program and/or a Licensed Agency.
Strong team building skills.
Knowledge of New York State Department of Health Codes and Regulations and Joint Commission standards.
Vehicle and current New York State driver’s license preferred.
Excellent organizational and presentation skills.
Basic computer skills.
