Record Analysis identifying, reporting, and monitoring the patient record for completion by providers prior to the record being considered complete and compliant according to facility and regulatory agencies.
Deficiency Monitoring Surveillance and Validation of Deficiency reports prior to provider incomplete notification.
Delinquency Process Timely Provider notification of incomplete records
Release of Information Assessing validity of requests for patient information, documenting/logging the request and compliance with releasing the patient information.
EMR Integrity - Accurate, complete and comprehensive documentation on the correct patient and correct encounter to document the patient’s treatment, response to treatment, outcomes of treatment, etc.
DNFB/DNFC Assistance Re-analysis of missing documents, routine communication to HIM leaders, providers, and clinical departments to obtain required documentation timely.
Registry Maintenance (e.g., Birth Registry) Accurately complete required registry information and submit to appropriate agency as required.
Scanning/Imaging Basic knowledge of content and location of documents in EHR.
Other specified duties i.e. Management of Transition of Care documentation and Federal Payer requirements (VA).
High School diploma or Equivalent
Knowledge and organizational ability to review medical records for completeness by following BMC/HIS guidelines
Ability to learn medical terminology
Proficient with Microsoft Office applications: Word, Excel, Office, PowerPoint, cloud and web based functionality.
Ability to pay close attention to detail to detect missing and/or incorrect information in the medical record and must also have the knowledge and skills needed to make the corrections when appropriate
Ability to maintain production standards as set forth by the HIM Operational Manager
Experience in an office environment, 1 -2 years in a health care setting or hospital is preferred.
Associate degree, rudimentary life science education to include medical terminology preferred
See, read, and/or operate computers, telephones, office equipment, documents, labels, including manipulating paper requiring the ability to move fingers and hands.
Remain sitting, standing, or walking for long periods of time to perform work on a computer, telephone, or other equipment.
Frequent interactions with associates, patient care providers, patients, and visitors that require associate to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information banding patients, etc.
Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items (over 50 lbs.).
